Medical Records

Every patient registered with a GP practice has the legal right to access their medical records for free.

Your records may include appointment history, test results, referral or hospital discharge letters, vaccination or childhood immunisations.

You can also nominate someone you trust to have access to your records if you need to.

Records are available digitally through the NHS App or via paper copies, which can be requested by downloading and filling in the access to medical records form and handing it in to either of our receptions.
