We ask that you always keep us up to date with your contact details and address, this ensures that we can communicate important information with you with ease. It also ensures that you are informed regarding your care.

If you move address, get a new phone number, update your email address or even change your name, please inform us of this.

You can update your details by submitting an Accurx via the following link and selecting the option for ‘Sick notes, test results or administrative help’.

We will contact you within 2 working days to acknowledge your Accurx.

Please note, if you are updating your home address, and your new address is outside of our catchment area, wee will advise that you will need to register with a new GP surgery closer to your home. Once we have been informed that you no longer live in our catchment area, you will be deducted from our service in 28 days. You will be deducted from our service sooner if you register with a new doctors before the 28 day deadline.
