The NHS offers a wealth of digital services for primary care.

Contacting Doctors and Nurses Online

Our online consultation Systmconnect, can be accessed from any internet-connected device. You can check your symptoms, get self-help advice or send a message directly to the practice team.

Systmconnect is quick, secure and easy to use. Many patients enjoy the easy access, flexibility and quick resolutions it offers.

A clinician may invite you to book an appointment or suggest another form of treatment. Often, our team can resolve simple issues without you needing to visit the surgery.

The NHS App

Free to download to your smartphone or tablet, the NHS App makes accessing primary health care services accessible.

The NHS App puts you in control, from ordering repeat prescriptions and booking appointments to getting self-help advice and displaying your vaccination status.

Proxy Access

Proxy access was developed to allow someone other than the patient to access their GP online services account. The proxy is given their own online access account (rather than using the patient’s login details). It is often used by the parents or recognised carers of young children, and recognised carers of adults.

Proxy Access for under 11’s

Proxy access can be granted by the guardian completing the CONSENT TO PROXY ACCESS TO GP ONLINE SERVICES form, they will need to provide 2 forms of ID including photo ID and a proof of address.

Proxy Access for adults 16+

Patients have the right to grant a proxy user or if the adult lacks capacity, access can be granted to a proxy acting in their best interests. A CONSENT TO PROXY ACCESS TO GP ONLINE SERVICES form will need to be completed and proof of ID and address will need to be seen.

Access for 11-16 year old’s

Patients requesting online access will need to book a routine appointment with a GP. This appointment is not urgent, therefore urgent ‘on the day’ appointments can not be used for this. Please be aware that routine appointments may not be currently available at your time of requesting, therefore you will need to check again in the near future to check routine appointment availability. When a patient of this age requests access, they will need to complete the APPLICATION FORM FOR ACCESS TO PATIENT ONLINE SERVICES form and make a routine appointment.

Disagreement of information on medical records

If you disagree with some information on your medical records, you will need to complete the PATIENT MEDICAL RECORDS COMMENT FORM. Once this form has been handed back to the surgery, this will be passed to management and a response will be made within one calendar month.
