Primary Care Networks

Thornfield Medical Group is part of the Newcastle East Primary Care Network (PCN). The PCN practices are: Thornfield Medical Group, Heaton Road Surgery, St Antony's Health Centre, Walker Medical Group, Benfield Park Medical Group, Biddlestone Health Group and Newcastle Medical Centre.

The PCN has been working collaboratively with the other member practices to identify health inequalities and how we can best support the local population.

The PCN way of working has enabled us to increase/ change our workforce roles to support our team to deliver improved services and have the roles oulined here to assist us.

Clinical Pharmacists

These pharmacists are highly qualified experts in medicines and can help people in a range of ways. Clinical pharmacists work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.

Having clinical pharmacists in GP practices means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time.

Social Prescribing Link Workers

These team members link patients to support networks in the community to improve wellbeing needs. They help to empower patients to gain confidence and self-esteem to allow them to live a more independent and healthy lifestyle.

Care Coordinators

Care coordinators are available to answer your queries and offer help and advice to ensure you receive care from the most appropriate part of the NHS, other care providers/ organisations to meet your health needs. They work to ensure patients are offered appropriate health appointments or screening for their age or health condition. Through close working with our medical teams they ensure NHS health needs are met according to personal health circumstances.

Care coordinators also work with social prescribing link workers to further enhance non-medical care or advice which is available to improve patients' lives. The care coordinators role is evolving to meet the demands of the services we provide.
